SERVICES and FEES
Park Your Headache is a Direct Pay Model, where the Patient pays the Practice directly for services rendered by the Practice, bypassing insurance and the need for costs of premiums, copays, deductibles.
Park Your Headache does not accept Medicare or Medicaid, and is “out of network” for all insurances. Please refer to “FAQs/Policies” page for more information.

Upon New Patient visit completion and if clinically indicated, the Practice is ready to administer injection treatment to provide immediate relief without added delays.
SERVICES:
New Patient (60 minute visit): Comprehensive and detailed initial evaluation
All patients must first complete a new patient evaluation with this Practice in order to receive any injections. This is regardless if the Patient is known to Dr. Park previously. Upon completion of new patient evaluation, Practice will determine if a primary headache disorder(s) exists to be treated, and if the patient is indeed clinically appropriate to continue with the Practice. Otherwise, recommendations for providers in clinically pertinent specialties will be provided.
Established Patient (30 minute visit): Follow up once patient has been established to the Practice, after completion of a New Patient visit
Follow up visits may be required as needed, to address in-depth concerns and/or updates that may arise, and/or to maintain and optimize outcome, and/or for treatment adjustments, etc.
Practice generally does not require more than yearly or bi-yearly follow up visits for clinically stable patients who only need basic check-in and maintenance prescription refills. These visits are available for the Patient as needed, and required to address updates in clinical issues and/or changes, should these arise.
Same-Day Established Patient (30 minute visit): Same day follow up after a patient has been established to the Practice, after the completion of a New Patient visit; mainly to address acute issues that may arise on short notice, contingent on Practice’s ability to accommodate on same day’s schedule
Occipital nerve block (ONB): Comprehensive injections of anesthetic medication administered to key hubs of nerves in the back of the head, which may help to “reset”chronic/acute migraine, break an active migraine attack or cycle, and/or may provide immediate relief
Trigger point injection (TPI): Comprehensive injections of anesthetic medication administered to myofascial spasms in the back of the neck and shoulders, which may help to increase overall range of motion and provide relief from tightness immediately
Botox injection for chronic migraine: will be offered soon, stay tuned for any announcements
Sphenopalatine Ganglion Block: will be offered soon, stay tuned for any announcements
FEES:
New Patient (60 minute visit): $345 (initial payment to be paid by credit card, cost is inclusive of credit card processing fee)
Established Patient (follow up; 30 minute visit): $180*
Same-Day Established Patient (same day follow up; 30 minute visit): $230*
Occipital nerve block (ONB): $375*
Trigger point injection (TPI): $375*
*Starred prices listed above are for cash/check payments. Current credit card processing fee is an additional 2.99% per transaction, which would be thenceforth added to the cash cost of service as listed, other than the New Patient visit and cancellation/late costs, which already include this processing fee.
Prices are subject to change any time by the Practice. Practice is committed to transparency and will immediately update this website upon any change
Credit card processing fee is determined by the credit card processing system
The Practice understands that unforeseen circumstances may occur and will do its best to reasonably accommodate. However, the following cancellation, late, no show/no call fee policies are in place in fair respectful consideration for the time of other scheduled and prospective Patients, and the Practice.
Appointment cancellation/late fee: $50 (charged to the Patient’s card on file, cost is inclusive of credit card processing fee),
Patients will be charged a cancellation fee if the appointment is cancelled less than 24 hours before the scheduled time.
Patients will be charged a late fee if Patient is considerably late without notifying the Practice.
No show/no call fee: 75% of the cost of the scheduled visit*
