HEADACHES AT BAY MEMBERSHIP PLAN:

For $99/month (inclusive of credit card fees), Patient is eligible to access 50% off of services offered by the Practice (“Services and Fees”). This is with the exception of the New Patient visit, which is initially required.

Headaches at Bay membership plan is an additional option that can overall help streamline fees and optimize continuity of care for the Patient. Patient may qualify for this plan if after initial completion of the New Patient visit, Patient is indeed deemed clinically appropriate to return to the Practice on a recurrent basis, including injection treatments. This is offered per Practice discretion.

Effective headache management is typically recurrent in nature over time. Whether the Patient decides to participate in the membership plan or not, is entirely up to the Patient with no obligation. The itemized Services and Fees listed on Website, are available as default pricing.

The membership plan has a non-refundable first month payment of $99, which will be subsequently followed by the same amount fee due on a monthly basis thereafter.

Participation in the monthly membership plan solely offers the Patient the additional option of access 50% off on “Services and Fees”, for streamlined fees over time.

Patient may cancel this membership plan at any time and for any reason, upon providing at least 30 day advance written notice to Practice. Cancellation request should be emailed to info@parkyourheadachenj.com.
